Jaklin Hanna, PharmD, BCACP is a pharmacist who specializes in ambulatory care clinical pharmacy, including chronic disease management. She pursued a career in healthcare because she finds it rewarding to build long-term relationships with patients, helping them address their medical needs and lead a healthier life.

Dr. Hanna is very passionate about diabetes, including the fast-changing diabetes pharmacotherapy and technology, as well as nutrition and lifestyle. She is a state-certified pharmacist immunizer, and board-certified ambulatory care pharmacist (BCACP). 

“I strongly believe in educating and empowering the patient to make informed decisions about their treatment and to take a leading role in the management of their own health, which fosters a trusting relationship with the provider and drives adherence to treatment plans”, she says.

Dr. Hanna feels it is most important during a patient visit is to actively listen to accurately assess patient’s understanding of their disease state and to identify and address any concerns. She enjoys seeing the patients' long-standing conditions improve with individualized management knowing that many serious complications have been prevented due to early interventions.

Outside of work, Dr. Hanna enjoys spending time with family, hiking with her dog Sphinx, and travelling. She is also passionate about healthcare access inequality, medication affordability and partaking in medical missions.
