Ms. Fani earned her Bachelor of Science in nursing from The Decker School of Nursing at Binghamton University in Binghamton, NY and received her Master of Science in nursing specializing in adult-gerontology primary care from Hunter College in New York, NY.

Prior to joining Summit Health (formerly Westmed Medical Group), Ms. Fani worked managing high-risk geriatric patients with the ALIGN team at the Brookdale Department of Geriatrics and Palliative Medicine at the Mount Sinai Hospital in New York City. During this time, she was heavily involved with projects to decrease ED utilization among the elderly population, guide end-of life discussions and decision making as well as helping to introduce palliative care to patients and their families.

Ms. Fani is board certified by the American Nurses Credentialing Center (ANCC) and is a member of the Sigma Theta Tau International Honor Society of Nursing.
